From Protostellar Disks to Planetary Systems

Note added March 10, 2006. We have reached the participant limit. Final participation by any of the pre-registered and approved persons is contingent on receipt of the registration fee (see below). New registrants will be placed on a wait list in case spaces open up again after April 17, 2006.

This workshop is limited to 40 participants. Please register by filling out and submitting the web registration form below. The deadline for submission of this form is March 15, 2006. Due to space limitations, participation cannot be guaranteed. We will contact you by email to let you know if your participation is approved.

If your participation is approved, your registration fee of $120 CDN or $110 US must be received by April 17, 2006 (please allow one week for delivery). The Goods and Services Tax (GST) is included in both amounts. Payment can be made in either Canadian or US bank cheques for the appropriate amount. Please mail your cheque (payable to The University of Western Ontario) to:

The registration fee covers coffee breaks (all day), lunches on May 18 and May 19, as well as the banquet on the evening of May 18.
